Job Description

About the role

The Specialist Support Coordinator forms part of a highly skilled team of allied health professionals in supporting children, young people and their caregivers to achieve optimal developmental outcomes. The position will be responsible for providing Level 2 and Level 3 Support Coordination and/or Therapeutic services specifically responsible for assisting clients and their families in achieving their goals and maximising their potential. This role will be based in our Elizabeth office and will require travel to various Kudos sites as required. 

Key Responsibilities: 

  • Manage a clinical caseload, including assessment, planning, and evaluation of NDIS plans. 

  • Assist clients in implementing their NDIS plans, engaging high-quality service providers. 

  • Coordinate complex care requirements, ensuring optimal support utilisation. 

  • Report and participate in NDIS client review processes. 

  • Maintain a caseload of 5 billable hours per workday. 

  • Encourage client and family participation in program evaluation. 

  • Engage support networks to resolve conflicts and build participant capacity. 

  • Evaluate interventions to meet goals and integrate with other Kudos services. 

  • Liaise with carers, families, and service providers to enhance quality of life. 

  • Contribute to NDIS service planning and reporting as required. 

What you look like’ and need to demonstrate to be successful’.  

  • Knowledge of best practice interventions, Strength-Based, and Child & Family-Centred practices. 

  • Excellent written communication skills, including high-quality report preparation. 

  • Experience working in multi-disciplinary teams to deliver individualised interventions for children with complex needs. 

  • Proven ability to maintain professional networks to promote best practices for children with disabilities. 

  • Strong interpersonal and collaboration skills, with the ability to work with people with disabilities, their families, and support networks. 

  • Ability to work autonomously and collaboratively in a team environment. 

  • Understanding of person-centred thinking, NDIS principles, and National Disability Services Standards. 

  • Relevant Graduate/Postgraduate qualifications for registration and practice as a Social Worker, Membership with the Australian Association of Social Workers (AASW) (Desirable)

It is a requirement of this position that the successful incumbent maintains a current Working with Children Check and NDIS Worker Screen Check which is satisfactory to Kudos in accordance with the Child Safety (Prohibited Persons) Act 2016 (SA).
